To complete your request for a Replacement OSHA card, please complete the form on this page. Replacement OSHA cards can only be processed by instructors whose name appears on your initial card. Please call 718-366-3590 before filling out this replacement request to determine if your initial instructor is associated with All NYC Safety. We can not process your request unless both payment and request form are received. If you have not made payment please click here to purchase the Replacement OSHA Card. Because the Replacement OSHA Cards are processed directly by OSHA, please allow 4-6 weeks to receive your replacement card. We will reach out to you if we need any further information during the verification. All training and identity must be verified before we issue a Replacement OSHA Card.
